Jessica has had a number of years experience working with issues around hospital discharge and supporting people in the community with long-term conditions, and can support you with various issues, for example:
- You have had a fall or accident
- You have had an operation eg hip or knee, abdominal, musculoskeletal, or any other surgery
- You have been unwell and are keen to get your strength up again
- You have a medical condition or long-term condition that causes you difficulty
- You have been into hospital a number of times recently and are keen to get on top of your health
- You are not sure how to link up to local NHS services in your area, or what you might be entitled to
- You are keen to build up your walking or mobility, or learn how to use walking aids/ crutches / wheelchairs etc
- You are a carer of someone you are concerned about
- You would like to know how to get out and about, or what local clubs or facilities there are
- You would like to access NHS rehabilitation services but need to know what’s available and how to do that
- You would like to understand your health better and try to prevent problems arising in future
- You are keen to get as healthy as possible
- You would like more physiotherapy or longer sessions than the NHS might offer
- You are going in to hospital and would like to be prepared and as strong as you can be
- You have any other concerns with your health or hospital admission
